چکیده

important sign of ill health in clinical medicine. It may occur in the lungs (pulmonary oedema), the abdominal cavity (ascites) and other body cavities (synovial, pericardial and pleural effusions) but in this article only peripheral (subcutaneous) oedema is discussed. In medical practice peripheral oedema tends to get pigeonholed according to possible systemic or peripheral causes eg heart failure, nephrotic syndrome, venous obstruction or lymphoedema. This viewpoint fails to appreciate the many dynamic physiological forces contributing to oedema development and in particular the central role of the lymphatic drainage system in tissue fluid balance. Consequently, the clinician’s approach to chronic oedema is often misguided and the necessary medical intervention inappropriate, for example, empirical use of diuretics. In this article we propose a system for managing peripheral oedema, which is based on physiological principles, that can then guide treatment. Why is chronic oedema important?

The Pathophysiological Aspects of Lymphology of Critical Conditions

In work on Wistar rats with experimental crush syndrome and acute alcohol intoxication shown that alterations of «metabolic profile» are stipulated by the lymphatic system, which emerges as an active component to urgent adaptation in critical situations. It is shown by the expressed infringements of an exchange of electrolytes in plasma. lymph and myocardium at the rats, that can make a part of...

Congenital Hereditary Lymphoedema in the Dog * Part I . Clinical and Genetic Studies

Congenital hereditary lymphoedema has been reported in man (Milroy, 1892; Esterly, 1965), cattle (Donald, Deas, and Wilson, 1952; Morris, Blood, Sidman, Steel, and Whittem, 1954), and swine (Wiesner, 1960). In each species, oedema of variable degree involves primarily the extremities and may have little effect upon general health. The underlying cause of the oedema is poorly understood. This re...

Comparison between interleukine-17 levels by NK-T cells in patients with chronic hepatitis C and healthy individuals

Background and Aims: Natural killer T (NKT) cells have been suggested to play critical roles in a wide range of immune responses especially against hepatotropic pathogens such as hepatitis C virus (HCV). In  the  present  study,  we  investigated  the  status  of  NKT  cells  by producing interleukin-17 cytokine in  peripheral  bl...

Diuretic resistant oedema: nailing the diagnosis.

Peripheral oedema is a commonly encountered clinical problem. Although it has many causes, its pathophysiology ultimately involves two processes: altered capillary haemodynamics and renal salt and water retention. A minority of cases are resistant to diuretic treatment; causes of such resistance include excessive sodium intake, poor diuretic drug absorption, reduced secretion of free diuretic i...

Chronic diffuse oedema of the rheumatoid hand: a sign of local lymphatic involvement.

Patients with rheumatoid arthritis sometimes develop chronic oedema of the hand, in no way related to the involvement of finger and wrist joints, and affecting the whole hand diffusely. This swelling, in our experience, is relatively resistant to therapy, though some result may be obtained by diuretics and corticoids. Since we have not found any reports of studies of the aetiological mechanism ...
